I've had this issue that tended to come and go where the 2nd probe of the fixed touch plate for a tool change would fail. I tried various things but I would still have the issue now and then. It would get worse the slower I would make the 2nd probe feed rate. Setting it to 2 would almost always cause a problem, while having it at 10 it would never have an issue.
A couple weeks ago I hit upon a fix that so far has made this problem a thing of the past. My acceleration for the Z axis was set rather low, (in my case 2). I bumped it up to 14. I've had the slow feed rate set to 2 and with over 100 tool changes, never a glitch.
I'm not going to speculate on why this might be, but it helped me so I wanted to share.
